Home healthcare

HRHHH offers home health care services for patients who wish to remain at home. In this program, patients are assigned a case manager, registered nurse, and other medical professionals as their care demands. After a health assessment, the case manager works with the patient and their family to create a treatment plan. At this point, nurses, speech therapists, and medical social workers may be consulted. The home care team will plan attentive care intended to improve the patient’s condition and avoid hospitalization. They may also assist the patient with daily tasks such as bathing, dressing, and preparing food.

Hospice care

It is difficult for anyone to encounter a life-limiting illness. HRHHH’s hospice care aims to ease the burden as much as possible. Hospice patients are granted access to around-the-clock care to ease their pain and suffering. In order to keep staff available for this demanding work, this program is limited to patients expected to live 6 months or less.

While the sick are given access to quality care, their families are offered bereavement counseling services to help them through this challenging time. This includes short-term counseling, access to support groups, and leads to other community resources for grief counseling.

Advance medical directives

Given the tenuous nature of life-threatening illnesses, patients are encouraged to establish advance medical directives to plan for contingencies. These directives allow the patient to appoint a trusted individual to make decisions regarding their treatment should they become incapacitated or mentally incompetent. Everyone has a right to receive treatment according to their desires, even when incapable of making those decisions themselves.
